[MassHistPres] Historical Structure or Building Dismantle Notification Form

Lancaster Historical Commission has not been notified of dismantling a building until it is too late to accomplish much as the building inspector form for Demolition does not cover Dismantle of a structure.
A few years back an late 1700's Inn was dismantled and moved to Ohio. Recently an old 1800s building used as a museum (formerly a residence which was moved onto the property for a previous resident where he displayed antique toys and doll houses prior to his death. We read in the local press is was being dismantled and moved to NY by its present owners. Our Commission would like to suggest a revised form with some guidelines helpful to all in communicating significant changes, if someone applies to dismantle a historic building. Our town voted down a local historic district twice.
Does any town have such a form that you would forward to us so that we could pursue such an informational process?
