[MassHistPres] Registering a new district with the Registry of Deeds?

All,

Does anyone know the exact procedure for amending a deed to show that it is
in a 40C Local Historic District?

E.g. do I get a signed document from the City Clerk with his seal showing
the exact boundaries of the district on an assessors' map?
