[MassHistPres] Examples-Demolition Delay Applications and Decisions

I've attached examples of our published Demo Delay Decisions. Since Hopkinton uses the Demolition Application to implement the Historic Significance review, we have no other application form.
I've also included the form for non-applicability regarding Historical Significance where the Historical Commission has determined that, while the structure is eligible for review as being more than 75 years old, the HHC (or its Chairman if a meeting can't be convened in 14 days) determines the property is not historically significant.
