[MassHistPres] NHPA/NEPA/Section 106 questionnaires

Hello,

Here is a link for a questionnaire given out to grant awardees by the National Endowment for the Arts related to NHPA/NEPA. https://www.arts.gov/sites/default/files/NHPA-NEPA-Questionnaire-PRA-11.1.19-rev.pdf

I wonder if anyone is familiar with other organizations/agencies/etc. who have similar NHPA/NEPA/Section 106 questionnaires that are sent out? Particularly those who work with individuals/organizations/municipalities who might not be aware of NHPA/NEPA/Section 106, and thus, this might be the first time this type of information is being flagged for them.

I am working on compiling a list of those who use these types of questionnaires when supporting different projects and figured I'd reach out to this group's wealth of knowledge.
